This notice describes the information this website may collect, how inquiry records and Help Hub plans work, and the boundaries that protect visitors from accidentally submitting sensitive clinical information.
Review before launch
Public governance Do not submit protected health information through general website forms.
Do not send diagnoses, treatment histories, medications, Social Security numbers, government ID images, insurance cards, court records, passwords, or emergency details. Website forms are for general contact, referral interest, partnership, workforce, employer, support, kitchen, and media inquiries.
1. Information we may collect
When you submit a website form, we may collect the fields you choose to provide, such as your name, email address, telephone number, organization, inquiry type, preferred contact method, and message.
The website may also process ordinary technical information necessary to deliver and secure the site, such as browser type, device type, request time, referring page, and security logs. The theme's inquiry rate limit uses a short-lived one-way hash derived from the network address presented to the server; the form record does not intentionally store the raw address.

2. How we use information
We use information to review and respond to inquiries, route requests, document follow-up, improve the website, protect security, meet legal or records obligations, and maintain evidence that a requested next step was addressed.
Submitting a form does not create a clinical relationship, employment relationship, partnership, contract, enrollment, or guarantee of services.
3. Inquiry form storage
The theme stores submitted inquiries as private records inside the site's WordPress database so authorized administrators can review and document follow-up. These records are not public posts.
The theme includes a configurable retention period and scheduled cleanup process. The organization must select and document a retention period appropriate to its legal, operational, privacy, and records obligations.
4. Help Hub plans and saved resources
The Help Hub uses browser local storage to save resources, checklist items, and a personal action plan on the device being used. That plan is not sent to The Open Table Cincy by the theme. Clearing browser data, using private browsing, or changing devices may remove the plan.
People who share a device should consider whether locally saved planning information may be visible to another user.
5. Cookies and third-party links
WordPress, security tools, hosting providers, analytics tools, embedded media, or other approved services may use cookies or similar technologies. The organization should maintain a current inventory of active tools and update this notice when website technology changes.
The Help Hub links to independent organizations. Their websites and privacy practices are governed by their own notices. A link does not mean The Open Table controls, guarantees, or endorses every service detail.
6. Sharing and sale
The Open Table Cincy does not intend to sell personal information collected through these theme forms. Information may be shared with authorized personnel and service providers who need it to operate, secure, host, maintain, or respond through the website, and when disclosure is required or permitted by law.
Referral or partner information should be shared only on a minimum-necessary basis and with appropriate consent or another valid authority.
7. Access, correction, deletion, and questions
You may contact The Open Table Cincy to ask whether the website holds an inquiry associated with your email address, request correction, or request deletion where applicable. WordPress privacy tools can export or erase inquiry records associated with an email address, subject to legal, safety, contract, or record-preservation exceptions.
Privacy requests must be verified before information is released or deleted.
8. Children and emergencies
This general website is not designed to collect sensitive personal information directly from children. A parent, guardian, authorized professional, or approved youth process should be used where youth information is necessary.
Website forms are not emergency channels. Call 911 for immediate danger and call or text 988 for mental health or suicide crisis support in the United States.
Launch review required
Before publication, leadership should insert the legal entity name, effective date, privacy contact, hosting/analytics/cookie inventory, retention rule, and any state-specific disclosures required for the actual website configuration.
